GOV.UK’s page footer asks what the visitor was doing and what went wrong. Fillo keeps those two intents and adds a visible page reference for a shared form. It is a short intake, not a full engineering bug report.

Route the task and problem together to the page or product owner. Voluntary reports cannot tell you how common the problem is.
